True Lateral View
Establish pelvic reference. Superimpose iliac cortical margins and sciatic notches to confirm patient positioning and landmark alignment.
Inlet View (20–30° Caudal Tilt)
Define the anterior sacral boundary. Align the S1–S2 disc cortical line to establish safe working corridor depth.
Oblique Inlet Working View
Optimize visualization for trajectory selection. Final working view confirms implant alignment before and during placement.
Precision at every depth
Dock the needle at the inferior SI joint third, approximately 10 mm from the anterior pelvic brim. Advance the cannula 6–8 mm beyond the posterior joint margin. Avoid malleting if misaligned.
First Allograft
Position 6–8 mm below the posterior joint margin with fluoroscopic confirmation on working view.
Second Allograft
Place at the middle-to-posterior third junction. Maintain a minimum 5 mm spacing between implants.
Controlled Advancement
Prevent excessive depth with cannula stops. Confirm each step on inlet and oblique inlet views before proceeding.
